As famed physicist Joseph Henry stated: &#8220;<em>The seeds of great discoveries are constantly floating around us, but they only take root in minds well-prepared to receive them.<\/em>&#8220;[\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]Maybe the history of the <a href=\"https:\/\/www.fratnow.com\/\" target=\"_blank\" rel=\"noopener\">FRAT<sup>\u00ae<\/sup> (Folate Receptor Autoantibody Test)<\/a> falls into a similar category. Its history and development are certainly fascinating! [\/vc_column_text][\/vc_column][\/vc_row][vc_row][vc_column][vc_custom_heading text=&#8221;History of the Folate Receptor Autoantibody Test (FRAT<sup>\u00ae<\/sup>)&#8221; use_theme_fonts=&#8221;yes&#8221;][vc_custom_heading text=&#8221;1. Origins and Discovery (2000s)&#8221; font_container=&#8221;tag:h3|text_align:left&#8221; use_theme_fonts=&#8221;yes&#8221;][vc_column_text single_style=&#8221;&#8221;]<\/p>\n<p>The FRAT<sup>\u00ae<\/sup> test emerged from decades long research by <strong>Dr. Edward Quadros<\/strong> and his colleagues at <strong>SUNY Downstate Medical Center<\/strong>, who investigated the role of folate in neurodevelopment and the impact of autoantibodies on folate transport, together with Dr. Vincent Raemakers from Belgium.<\/p>\n<p>[\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]The question Dr. Quadro\u2019s lab was looking to answer was quite straightforward but also very complicated. The food supply in the United States is fortified with folic acid (a synthetic B-9 vitamin), in order to prevent <a href=\"https:\/\/autism.fratnow.com\/blog\/folate-folate-receptor-autoantibodies-neural-tube-defects\/\" target=\"_blank\" rel=\"noopener\">neural tube defects<\/a>; so why is it that women were still having children with neural tube defects? Neural tube defects did decrease after food fortification for folic acid, however, there was still a large among of NTD births post-folate fortification. What in the folate cycle\/folate metabolism was causing this? Something was certainly amiss.<br \/>\n[\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]With this question in mind, Dr. Edward Quadros came across the first sampling of folate receptor autoantibodies. <a href=\"https:\/\/autism.fratnow.com\/blog\/folate-receptor-autoantibodies-in-families-with-autistic-individuals\/\" target=\"_blank\" rel=\"noopener\">Folate receptor autoantibodies (FRAAs)<\/a> were first identified via the FRAT<sup>\u00ae<\/sup> test in women with a history of neural tube defect (NTD) pregnancies, suggesting an autoimmune mechanism disrupting folate delivery to the fetus. This was a remarkable finding and published in the New England Journal of Medicine. A new condition contributing to neural tube defects was discovered![\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]The next step was to look for these folate receptor autoantibodies in children, when the same group came across a large number of children that were regressing in their development. Why was this the case? It seemed that they were developing typically for the first several months after birth, when, out of nowhere, there were major regressions in their development. Symptoms that suddenly occurred without warning included: [\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]<\/p>\n<ul class=\"mr-left-ul-40\">\n<li><strong>Cognitive impairmen<\/strong>t (intellectual disability)<\/li>\n<li><strong>Movement disorders<\/strong> (ataxia, dystonia, chorea)<\/li>\n<li><strong>Seizures<\/strong> (epilepsy)<\/li>\n<li><strong>Speech difficulties<\/strong> (delayed or absent speech)<\/li>\n<li><strong>Autism-like behaviors<\/strong> (social withdrawal, repetitive behaviors)<\/li>\n<li><strong>Sleep disturbances<\/strong><\/li>\n<li><strong>Microcephaly<\/strong> (in some cases)<\/li>\n<li><strong>Peripheral<\/strong> neuropathy (tingling, weakness)<\/li>\n<\/ul>\n<p>[\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]Basically, this was a developmental delay \/ regression of some sort (loss of previously acquired skills) with no explained reasoning. [\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]Again, in these cases, FRAAs were linked to what was now going to be defined as <a href=\"https:\/\/autism.fratnow.com\/blog\/cerebral-folate-deficiency-an-overview\/\" target=\"_blank\" rel=\"noopener\"><strong>cerebral folate deficiency (CFD)<\/strong><\/a>, which overlapped with symptoms of <a href=\"https:\/\/autism.fratnow.com\/blog\/a-comprehensive-introduction-to-autism-spectrum-disorder\/\" target=\"_blank\" rel=\"noopener\"><strong>autism spectrum disorder (ASD)<\/strong><\/a>. Approximately 70% of children with ASD were testing positive for these antibodies. This was an unexpected and exciting discovery! Again, it seemed that a blockage of folate transport by these autoantibodies was the culprit.<br \/>\n[\/vc_column_text][\/vc_column][\/vc_row][vc_row][vc_column][vc_custom_heading text=&#8221;2. Development of the FRAT<sup>\u00ae<\/sup> (2010s)&#8221; font_container=&#8221;tag:h3|text_align:left&#8221; use_theme_fonts=&#8221;yes&#8221;][vc_column_text single_style=&#8221;&#8221;]With the discovery of FRAAs in neural tube defects and cerebral folate deficiency, (overlapping with ASD symptoms), it was imperative to develop a commercially available assay (test) that would be able to screen for folate receptor autoantibodies on a mass scale. Again, the purpose was to detect FRAAs via a simple <strong>non-invasive blood test<\/strong>, replacing the need for any lumbar punctures to measure cerebrospinal fluid (CSF) folate levels.<br \/>\n[\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]Licensed by the <strong>Research Foundation for SUNY<\/strong> to <strong>Religen Inc<\/strong>, a company focused on neurodevelopmental disorder diagnostics, the test was validated in their <strong>CLIA-certified labs<\/strong> and made available globally to the public in 2016. As the test gained prominence, there were other areas of neuropsychiatry and neurodevelopment disorders where FRAT<sup>\u00ae<\/sup> was applied. This included:[\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]<\/p>\n<ul class=\"mr-left-ul-40\">\n<li>Schizophrenia<\/li>\n<li>Depression<\/li>\n<li>Epilepsy<\/li>\n<li>Subfertility<\/li>\n<li>Multiple Sclerosis<\/li>\n<li>Parkinson&#8217;s Disease<\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column][\/vc_row][vc_row][vc_column][vc_custom_heading text=&#8221;3. Key Scientific Milestones&#8221; font_container=&#8221;tag:h3|text_align:left&#8221; use_theme_fonts=&#8221;yes&#8221;][vc_column_text single_style=&#8221;&#8221;]Along with the development of the FRAT<sup>\u00ae<\/sup> test, there were other major key scientific milestones which bolstered the science behind folate receptor autoantibodies and their implications in neuropsychiatric and neurodevelopmental disorders.[\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]<\/p>\n<ul class=\"mr-left-ul-40\">\n<li><strong>Animal Models:<\/strong> Studies in rats showed that FRAAs exposure during pregnancy caused fetal brain malformations and behavioral deficits, reversible with <strong>folinic acid<\/strong> treatment.<\/li>\n<li><strong>Clinical Trials:<\/strong>\n<ul class=\"mr-left-ol-40 circle-list mr-left-ul-40\">\n<li>A 2012 study found that <strong>75% of children with ASD had FRAAs<\/strong>, and those treated with folinic acid showed improvements in language and social interaction8.<\/li>\n<li>Follow-up research confirmed FRAAs\u2019 role in <strong>subfertility, preterm birth, and neuropsychiatric disorders<\/strong> (e.g., schizophrenia, depression).<\/li>\n<li>FDA Clinical trials using FRAT<sup>\u00ae<\/sup> and treating with folinic acid are currently ongoing.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column][\/vc_row][vc_row][vc_column][vc_custom_heading text=&#8221;4. Current Applications and Impact&#8221; font_container=&#8221;tag:h3|text_align:left&#8221; use_theme_fonts=&#8221;yes&#8221;][vc_column_text single_style=&#8221;&#8221;]FRAT<sup>\u00ae<\/sup> has garnered worldwide use as it has become a practical diagnostic to indicate the presence of folate transport impediments. As such, physicians have had much success with its use and their subsequent treatment protocols after receiving FRAT<sup>\u00ae<\/sup> results.[\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]<\/p>\n<ul class=\"mr-left-ul-40\">\n<li><strong>Diagnostic Use:<\/strong> FRAT<sup>\u00ae<\/sup> is recommended for:\n<ul class=\"mr-left-ol-40 circle-list mr-left-ul-40\">\n<li>Children with <strong>ASD<\/strong>, developmental <strong>delays, or<\/strong> seizures.<\/li>\n<li>Women with <strong>a history of NTD pregnancies or subfertility.<\/strong><\/li>\n<li>Patients with treatment resistance depression, or depression in general.<\/li>\n<li>Patients with neurodevelopmental\/neurodegenerative disorders.<\/li>\n<\/ul>\n<\/li>\n<li><strong>Treatment Guidance:<\/strong> Positive results often lead to <strong>folinic acid therapy<\/strong>, which bypasses blocked folate receptors. This determination will be made by the physicians after FRAT<sup>\u00ae<\/sup> results are provided.<\/li>\n<\/ul>\n<p>[\/vc_column_text][\/vc_column][\/vc_row][vc_row][vc_column][vc_custom_heading text=&#8221;5. Future Directions&#8221; font_container=&#8221;tag:h3|text_align:left&#8221; use_theme_fonts=&#8221;yes&#8221;][vc_column_text single_style=&#8221;&#8221;]FRAT<sup>\u00ae<\/sup> is a fascinating test and so is the science behind it. Beyond its utility in identifying possible folate transport issues, it is a guideline for treatment that can and has benefited thousands upon thousands of patients. [\/vc_column_text][vc_column_text single_style=&#8221;&#8221;]<\/p>\n<ul class=\"mr-left-ul-40\">\n<li>Research continues on:\n<ul class=\"mr-left-ol-40 circle-list mr-left-ul-40\">\n<li><strong>Maternal FRAAs screening<\/strong> to prevent developmental disorders.<\/li>\n<li><strong>Dietary interventions<\/strong> (e.g., dairy avoidance) to reduce antibody production.<\/li>\n<li><strong>Genetic and environmental factors<\/strong> influencing FRAAs prevalence.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>[\/vc_column_text][vc_column_text
